What Causes Plantar Fasciitis?
Plantar fasciitis develops when the plantar fascia is chronically overloaded. Several factors independently or in combination create the conditions for this overloading.
Flat feet and overpronation are among the most common structural contributors. When the arch collapses excessively during the stance phase of walking, the plantar fascia is stretched beyond its designed range of motion with every step. Over time, this repetitive overstretching creates the cumulative micro-damage that leads to inflammation. If you have low arch or flat feet, you are at significantly elevated risk for plantar fasciitis.
High arches create a different but equally problematic loading pattern. A foot with a rigid high arch has poor shock absorption because the arch cannot flex sufficiently to distribute impact forces. The result is excessive concentration of force at the heel and forefoot, which overloads the plantar fascia from the opposite end of the biomechanical spectrum. People with high arches are often surprised to learn they are also at risk for plantar fasciitis.
Extended standing or walking on hard surfaces is a major occupational and lifestyle factor. Healthcare workers, teachers, retail employees, warehouse workers, and anyone else who spends most of their workday on their feet on hard floors subjects their plantar fascia to sustained loading that accumulates over time.
Sudden increases in activity are a classic trigger. Runners who significantly increase their weekly mileage too quickly, or someone who goes from a sedentary lifestyle to suddenly spending hours on their feet, overwhelm the plantar fascia's ability to adapt.
Footwear without adequate arch support is perhaps the most widespread and easily correctable contributing factor. Flat shoes, flip flops, worn-out sneakers, and fashion shoes that prioritize style over function all fail to support the arch and absorb impact adequately, placing the full burden of every footstrike on the plantar fascia itself.
The Classic Symptoms of Plantar Fasciitis
Plantar fasciitis has a very characteristic symptom pattern that distinguishes it from other foot conditions. The hallmark symptom is sharp heel pain with the first few steps in the morning. This happens because during sleep, the plantar fascia tightens and shortens. The sudden stretch of those first steps tears at the inflamed tissue, producing the characteristic morning pain spike.
Pain typically decreases after you have been moving for 5 to 10 minutes as the tissue warms up and loosens. It often returns after prolonged standing, after sitting for extended periods and then standing again, and at the end of a long day on your feet. Pain is usually localized to the bottom of the heel and the inner arch, and is typically worse with barefoot walking on hard surfaces.
